Form 1095-B is a tax form that shows proof of medical coverage. Initiated in 2016, most people have to report to the federal government that they, along with their dependents, had medical coverage during the previous year.
The Affordable Care Act is a federal law that requires almost everyone in the United States to have minimum essential coverage.
Yes, the retiree health plans offered under TRS-Care Standard meet the minimum essential coverage requirements because they are employer-sponsored coverage.
If you, your spouse, and/or dependent(s) had Traditional Medicare Part A at any time during the plan year, you'll receive a 1095-B from Medicare. If you haven't received your form by mid-February, please call Medicare at 1-800-633-4227.
If you, your spouse, and/or your dependent(s) are enrolled in TRS-Care Medicare Advantage with Part B only, and weren't enrolled in Medicare Part A at any time during the plan year (Jan. 1 – Dec. 31), you will receive a 1095-B from TRS.
TRS will mail 1095-B forms by mid-January. If you have not received your form by early February, contact TRS Health at 1-888-237-6762.
Use the form to verify medical coverage on your federal tax filing. You should keep Form 1095-B with all of your tax records as supporting documentation.
You can photo copy the original form for your dependents. If you did not receive the form, or you misplaced it, you can contact TRS Health at 1-888-237-6762 to request a duplicate form.
