Naming a Beneficiary
FAQs
- Log in to your MyTRS account and update the information under the Beneficiary tab.
- Active Members – Please complete a Designation of Beneficiary form (TRS 15) (pdf) and return it to TRS by mail or fax.
- Retirees – If you wish to change the beneficiary(ies) you listed for survivor benefits only, you can complete and return the Designation of Beneficiary form (TRS 15) (pdf) by mail or fax.
- If you selected Option 1, 2, or 5, you may only change your beneficiary for the continuing monthly payment in certain circumstances. For additional information, refer to the Post-Retirement Beneficiary and Payment Plan Changes section of the TRS Benefits Handbook (pdf).
- If you selected Option 3 or 4 and you wish to change your beneficiary(ies) for any remaining payments under the option, complete and return the Change of Beneficiary for Continuing Optional Retirement Annuity form (TRS 30D) (pdf).
Note: Sending a Designation of Beneficiary form (TRS 15) to TRS to request a change in beneficiary information will override any existing beneficiary forms on file. Also, sending a signed and dated blank form will result in no beneficiary designation.
- Active Members – You may add dependents during the plan year if you experience a special enrollment event, such as a marriage or the birth of a child.
- You must enroll the dependent within 31 days of the special enrollment event. The coverage takes effect the first of the month following the date of enrollment. Newborn coverage is effective on the date of birth.
- You also have an annual opportunity to add dependents during the summer enrollment period. You don't need a special enrollment event to add dependents during the Annual Enrollment.

- Retirees – You may be eligible to add a child as a dependent if you experience a special enrollment event. For additional information, refer to TRS-Care Eligibility and Enrollment or contact TRS Health & Insurance Benefits Eligibility and Enrollment by calling 1-888-237-6762.
Note: A child listed as a dependent is automatically terminated at the end of the month that they turn age 26. A notification letter is issued to the retiree prior to the termination of their dependent's health coverage.
- a natural child,
- an adopted child or one who is lawfully placed for adoption,
- a foster child,
- a stepchild, or
- a grandchild who lives with the participant, depends on the participant for support and is claimed by the member, retiree or surviving spouse for federal income tax purposes.
You may update your tax withholding information at any time through the MyTRS portal or by completing the Federal Income Tax Withholding form and returning it to TRS by mail or fax.
